Applications can be download here and are also available on-site at any participating school. Completed applications must be submitted it to any participating school no later than Friday, June 29.


When submitting your application at a participating school, you must show proper documentation that includes proof of residency and proof of income eligibility.

Proof of residency (bring any one of the following):     

• Rental lease or mortgage statement 
• Homestead Exemption notice for the 2011 calendar year
• Property tax notice for the 2011 calendar year
• Electricity/Gas or Sewage/Water bill
• Telephone bill
• Cable or internet service bill

Proof of income:  If you participate in any one of these Federal Assistance programs, you meet the income requirement of the scholarship program. When you turn in your application, please bring any one of the following:

• Louisiana Purchase Card
• SNAP Benefits
• Social Security Benefits

If you do not participate in a Federal Assistance programs, click here to determine if you meet income requirements. You will need to provide proof of income for every member of your household. This can consist of any of the following:

• Federal Tax Return for the period ended December 31, 2011
• W-2s from all employers for the period ended December 31, 2011
• Unemployment compensation statement for the period ended December 31, 2011
• 1099s and/or statements from banks or other institutions showing interest earnings for the period ended December 31, 2011
• Alimony as shown in court decree or agreement
• Social Security benefits statement for the period ended December 31, 2011
